BRAZILIAN JIU-JITSU WAIVER AND RELEASE OF LIABILITY
State of Flow Brazilian Jiu Jitsu Academy LLC– Marysville, Michigan
IMPORTANT: THIS IS A LEGAL DOCUMENT. PLEASE READ CAREFULLY.
In consideration of being allowed to participate in any way in Brazilian Jiu-Jitsu (BJJ), martial arts, self-defense training, fitness training, or related activities and events (“Activities”) at State of Flow BJJ (the “Gym”), I agree to the following:
1. Acknowledgment of Risks
I understand that participation in Brazilian Jiu-Jitsu and related Activities involves physical contact, strenuous activity, and the risk of serious injury, including but not limited to sprains, strains, dislocations, broken bones, concussions, or death. These risks may arise from my actions, the actions of others, training conditions, or equipment.
2. Voluntary Participation and Assumption of Risk
I voluntarily choose to participate and fully accept all risks, whether known or unknown, even those arising from the negligence of others. I affirm that I am in good physical condition and medically able to engage in training.
3. Release and Waiver of Liability
To the fullest extent allowed by Michigan law, I release and hold harmless State of Flow BJJ, its owners, instructors, staff, agents, volunteers, and representatives from any and all liability, claims, or damages arising from my participation in these Activities, including those arising from negligence.
4. Indemnification
I agree to indemnify and hold harmless all Released Parties against any claims, demands, or lawsuits resulting from my participation.
5. Medical Treatment Authorization
If I am injured or become ill, I authorize the Gym and its staff to seek medical care on my behalf and accept full responsibility for any costs related to that care.
6. Photo/Video Release
I give permission for State of Flow BJJ to take and use photos, video, or other media of me for promotional or instructional use without compensation.
7. Minor Participants
If I am signing for a minor (under 18), I certify that I am their parent or legal guardian. I agree to all terms of this waiver on the minor’s behalf and allow their participation.
8. Governing Law
This agreement is governed by the laws of the State of Michigan. Any legal matters shall be handled in St. Clair County, Michigan.
I HAVE READ AND UNDERSTAND THIS AGREEMENT. I AM SIGNING IT VOLUNTARILY AND KNOWINGLY WAIVING CERTAIN LEGAL RIGHTS.
